Watch your spacing. Looks like all through the Card you are missing the space after the colon, before the opening square bracket around the link. Also, you need spaces around the | when you have multiple links for one resource.

Also, and this is not required, but when you use a resource like National Cancer Institute multiple times, you can reference it as "National Cancer Institute (NCI)" the first time, then as simply "NCI" in subsequent references. This applies to any of the institutes/hospitals with long names that often go by acronyms (WHO, CDC, ACS, NINDS, etc).

- Please add some. The last sentence of your GuideNote will make a good one.
